Brightwell Pathways provides trained and experienced practitioners who deliver practical support at home, in the community and alongside education. Support is tailored to each child or young person’s needs, age and circumstances.

    STEP 1 - TALK TO US

    Start with an initial enquiry and consultation. We’ll listen to your concerns and discuss the support you’re looking for.

    STEP 2 - UNDERSTAND

    We'll learn more about your child, their needs, strengths and the areas where practical support could help.

    STEP 3 - PLAN THE SUPPORT

    We agree goals, the type of support needed and recommend the most appropriate support package for your child.

  • STEP 4 - DELIVER SUPPORT

    Your practitioner provides agreed support at home or in the community, based on the plan.

    We review progress with you, adjust support as needs change and identify next steps.

HOW WE SUPPORT YOUR CHILD

Every service is supported by a clear plan. Your practitioner keeps a record of each session and uses your child’s personalised support plan to record what has been delivered, monitor progress and capture important information. Where appropriate, risk assessments are also used to help keep support safe and well planned.

Information about your child’s support is kept securely on their file in accordance with UK data protection requirements and Brightwell’s data protection policies. This helps us review progress, maintain quality and make sure support continues to meet your child’s needs.
